Use this item on any card reader Return Label needed, for products purchased outside of the parts.getunion.com site. Ie. From original install, purchased before the Union Parts site was offered. 

For Return Labels for products purchased within the parts.getunion.com site, request a return for that part within your online account. 

READERS THAT ARE UNDER WARRANTY MUST BE RETURNED WITHIN 30 DAYS OF SHIPPING REPLACEMENT. REPLACED READERS NOT RETURNED WITHIN 30 DAYS OF SHIPPING NEW WILL BE INVOICED AT FULL PRICE PLUS SHIPPING.
